* NOTE: Page 1 and Part 1 Must be filled out. Part 2 may be filled out and signed by a parent/guardian or substituted with a current copy of a schools or doctors physical.

The player goals and skills sheet should be filled out by the player prior to arriving, we will have extra copies at registration and we will spend a little time in the morning reviewing the questions. Players will have time to fill it out at the field as well.

Team Discounts: Take $15 off for each team member or friend. (5 or more and you qualify. Registrations “must” be sent together.)

Berkshire County Lacrosse Association, (BCLA) Discount: Take $10 off if you played on one of the boys/girls teams in the BCLA this year.

Williamstown Lacrosse Association, (WLAX) Discount: Take $10 off if you played in the WLAX program this year, grades 1-8.

* Note: discounts can’t be combined with each other. We will apply the greater of the discounts.
